When complaints are received which suggest that there may be a systemic issue within a local authority, a school or any service provider for which the local authority is responsible, a local authority may wish to consider commissioning an investigation to explore the issue complained of and determine whether action is required. Such investigations often involve sensitive issues such as bullying, harassment, discrimination or a failure to respond appropriately to concerns raised by service users.
